Within the Sociaal Team Houten there is room for a new colleague to fill the varied and challenging position of Wmo Professional. The Social Team Houten is looking for a Wmo Professional who is specialized in investigating Wmo counseling requests and can also carry out counseling projects himself. Obviously you will look at questions of inhabitants of Houten from all living areas.

Foundation Social Team Houten

Stichting Sociaal Team Houten is the point of contact for prevention, support, guidance and care. The statutory tasks from the Wmo and Youth Act, aimed at prevention and support, have been placed in an independent foundation. Social Team Houten is an implementing organization of the municipality, in which the municipality has influence through the College of Mayor and Aldermen.

The foundation is characterized by its open atmosphere, collegiality and looking out for each other. You will end up in a place where committed professionals work and where both professional and personal development are central. There is always someone available to spar with and see what is needed. We are there for each other and celebrate successes, but we also find each other when things are not going so well.

Our vision

Our vision is that not everything that is different or goes differently than you would like or expect is a problem and immediately requires professional help or support. We want to help people cope with social, physical and emotional challenges in life and help them to take control of them themselves. If support is needed, we offer it on a tailor-made basis, as close to the home and own network as possible and as much as possible in line with normal life.

What will you do?

What will you do?

You support adults and their network with the challenges in life and the sustainable improvement of their situation. You can and dare to keep asking questions and have an eye for all areas of life, whereby it goes without saying that you start from the capabilities of residents. Own solutions are always preferred over help from professionals. You connect to the experiences and wishes of the resident. Together you make a plan and look for appropriate solutions, not losing sight of the Wmo framework. The goal of the support is for residents to regain control of their own lives. Important others, the network and informal care are involved. Where necessary you work together with other parties, stakeholders and facilities in the basic infrastructure (such as general practitioners, welfare consultants, psychologists, etc.).

You fulfill three roles;

  • Indicating - You examine applications and look broadly at the areas of life.
  • Supervise - You provide your own supervision as needed and encourage own strength and network.
  • Point of contact - You are visible and accessible at the Social Square.

You will use your own specialty, but will also be a generalist in recognizing and assessing when help from others should be called in.

Work
  • Conducting the investigation in various areas of life. This includes determining severity and risk, such as when a resident's safety is at stake;
  • Indicate specialized help, care and services, as needed;
  • Provide ambulatory counseling yourself;
  • Creating a plan of action;

  • Engaging social networks and informal care so that people can continue on their own as much as possible and with the help of those networks;
  • Collaborate as much as possible with the parties from the basic social infrastructure in Houten (formal and informal help);
  • Collaboration with various parties both from the 0e, 1e as 2e line;
  • Coordinate care in cases where more than one caregiver or agency is involved and those involved cannot do it themselves;
  • Evaluate and scale down care where possible and responsible. When problems have become manageable, basic services can take over support again;
Job Requirements
  • You have a completed relevant HBO or WO and have at least several years of experience;
  • You have an affinity for generalist work;
  • You believe in the power of people and their environment. You work solution-oriented and connect to what the resident needs without taking over. You help residents to call on their own network and, where necessary, you steer for behavioral change;
  • You are able to see beyond the boundaries of your own field;
  • You can deal with conflicting interests and loyalties and in this you are aware of your own standards and look at the standards of others with an open mind;
  • You can work independently, learning and reflectively in a team as well as individually;
  • You yourself know how to balance workload and job satisfaction.

What we offer

What we offer

  • You will have a contract for the duration of one year with the possibility of extension. Employment conditions are in accordance with the Collective Labor Agreement for Social Work. You will be placed on scale 9. The maximum salary as of January 1, 2025 is €5574,00 gross per month for 36 hours. You will be scaled according to your experience.
  • There is an Individual Choice Budget of approximately 19-21% (depends on grading) and a Career Budget of 1.5%.
  • You get your own laptop and phone.
